Flex Plans

Flexible spending plans can be used to cover the costs of some health and refractive services. Talk to your employer about setting aside money for you to put toward the costs of your treatment. It is a good idea to find out if you are a candidate before making such an election.

Insurance Accepted

Our insurance coordinators deal with many insurance companies and because many companies frequently change policies, it is sometimes difficult to accurately estimate our patients' insurance co-payment. We will do our best to provide you with an estimate of your out-of-pocket costs, but your insurance company has the final decision on what services they cover. Just as it is important for you to keep us informed about any changes to your medical history, it is also important that we know about changes to your insurance so that we are as accurate as possible.

Co-Payment: All payments (co-payments, co-insurance, deductibles, premium procedures, etc.) are due on or before the day of your surgery.

Medicare Patients: Medicare pays us directly for your care but you are responsible for any deductible and co-insurance. We will do our utmost to ensure coverage of all services. lf Medicare denies your claim, however, you are responsible for the charges.

Private and Group Insurance: As a courtesy, we will file your insurance claims for you. Upon receipt of payment for services rendered to you and an Explanation of Benefits (sometimes called an EOB) from your insurance provider, your account with Advanced Surgery Center will be reconciled. Our team will refund you any money you may have over-paid or will bill you if there is a balance due on your account. It is important to know that you will likely see three different EOBs for your surgery: one from Advanced Surgery Center, one from your operating surgeon, and one from the anesthesia provider.

HMO Patients: Most HMO patients must have a referral from their primary care physician. Obtaining a medical referral is the patient’s responsibility but we can help you should you need assistance.

For testing and procedures that are not covered under your medical or vision plan we have a variety of payment plans to help make your treatment more affordable. Please contact our surgical center and speak with one of our capable health care coordinators. We want to make it easy for you to address your medical and vision problems, and we will work with you to make that attainable.
